\n\t\t\t\t\t\t\tProllenium\u00ae<\/sup> Medical Technologies Inc. (\u201cProllenium\u201d, \u201cwe\u201d, or \u201cus\u201d<\/strong>) is the provider of the Prollenium\u00ae<\/sup> Rewards App (the \u201cApp\u201d<\/strong>). The Prollenium\u00ae<\/sup> Rewards App is an Android\/iOS app that can keep you informed about Prollenium\u00ae<\/sup> products, and can connect you to your local clinic to send you the latest offers and loyalty rewards on Prollenium\u00ae<\/sup> products and clinic services. With the Prollenium\u00ae<\/sup> Rewards App, every time you purchase a qualifying treatment with a participating provider you can earn credit towards a reward at your clinic.\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

\n\t\t\t\t\t\t\tThese terms of use (the \u201cTerms\u201d<\/strong>) govern your relationship with Prollenium\u00ae<\/sup>, your use of the App, and any Offers and Rewards you may receive through the App. By using the App, you agree to these Terms in their entirety. If you do not wish to be bound by these Terms, or if you are unable to agree for any reason, you cannot use the App.<\/strong>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

\n\t\t\t\t\t\t\tWe may update these Terms from time to time by posting an amended version of the Terms on our website at https:\/\/prollenium.com\/terms-of-use\/<\/strong><\/a>. \n\t\t\t\t\t\t\tIf at any time you decide that you no longer wish to be bound by these Terms, you must terminate your account by sending a message requesting termination of your account to Prollenium\u00ae<\/sup> through the App or at privacy@prollenium.com<\/strong><\/a>. Upon termination of your Account for any reason, any offers or rewards to which you may have been entitled will be permanently lost.\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

\n\t\t\t\t\t\t\tIf you wish to temporarily uninstall the App from your mobile device, you may do so at any time without terminating your account. Uninstalling the App will not affect your entitlement to any earned Offers or Rewards. However, you must reinstall the App (and provide your account login information) in order to earn or redeem Offers or Rewards.\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

\u201cAccount\u201d<\/strong> a user account created within the App.<\/div>\n
\u201cApp Content\u201d<\/strong> any and all text, image, computer code, script, information, Offer, Reward, or other data which is intended to be delivered to users by means of the App.<\/div>\n
\u201cApp Notification\u201d<\/strong> any message, notification, or alert that the App may cause to be displayed on a user\u2019s Device.<\/div>\n
\u201cApp\u201d<\/strong> the Prollenium Rewards App.<\/div>\n
\u201cClinic\u201d<\/strong> the medical clinic that you select as your designated local clinic when you register your account, as further defined in the\n
\u201cYour Designated Clinic\u201d<\/strong> section below.<\/div>\n
\u201cDevice\u201d<\/strong> any smartphone, tablet or other device used to access the App.<\/div>\n
\u201cOffer\u201d<\/strong> any offer or proposal made available to you through the App to provide you with some kind of reward, for example, an Offer may make a product or service available to you free of charge or at a discounted price; discount the price of a future purchase of products or services; or (3) provide you with free or discounted products. There may be terms and conditions attached to Offers.<\/div>\n
\u201cPassword\u201d<\/strong> the password used to access your Account.<\/div>\n
\u201cProllenium\u00ae<\/sup> Reward\u201d<\/strong> a Reward made available to you by Prollenium\u00ae<\/sup> as part of any Prollenium\u00ae<\/sup> Offer.<\/div>\n
\u201cProllenium\u00ae<\/sup> Offer\u201d<\/strong> any Offer made available to you by Prollenium\u00ae<\/sup>.<\/div>\n
\u201cClinic Reward\u201d<\/strong> a Reward made available to you by your Clinic as part of any Clinic Offer.<\/div>\n
\u201cClinic Offer\u201d<\/strong> any Offer made available to you by your Clinic.<\/div>\n
\u201cProllenium\u201d<\/strong> Prollenium\u00ae<\/sup> Medical Technologies Inc.<\/div>\n
\u201cReward\u201d<\/strong> any free or discounted product or service, or any other enticement that you may earn, redeem or become eligible to claim if you meet the terms of an Offer.<\/div>\n
